Colin Crowe, assistant professor at the School of Medicine, published an article titled "Connecting Patients with Diabetes to Pathways Community HUBs to Address Social Needs," in a December issue of Capsules, a publication from the Ohio Cardiovascular & Diabetes Health Collaborative (Cardi-OH).
Unmet social needs represent a major barrier to successful diabetes management and control. The article discusses how a referral to the Pathways Community HUB model can help address a patient’s social needs.
