What was the NEO QI Hub Produce Path Program?
The program provided eligible patients with $100 in coupons that could be used to buy fruits and vegetables at participating farmers markets.
Who was Eligible?
- Patients of participating clinical sites who were 18 and older and
- Patients who had Medicaid or Medicare insurance (without supplemental private insurance) or who were uninsured
How Did it Work?
- Participants visited the information booth at their doctor's office to sign up.
- If eligible for the program, participant received a booklet of coupons to shop at participating farmers markets through June 30, 2025.
What Did Participants Get When They Signed Up?
- $100 in coupons to buy fruits and vegetables at participating farmers markets
- A reusable shopping bag
- Get set up with the Produce Path mobile app
